Hospice Levels of Care
At Solace Valley Healthcare, we understand that each patient’s needs can change throughout their hospice journey. That’s why we offer four distinct Levels of Hospice Care—each designed to ensure that patients receive the appropriate support, when and where they need it most. These levels are part of the Medicare Hospice Benefit and are available to all eligible individuals.

Routine Home Care
This is the most common level of hospice care. It provides regular, scheduled visits from the hospice team to manage symptoms and offer support in the comfort of the patient’s home, whether that’s a private residence, assisted living facility, or nursing home.
Services include:
- Nursing visits for pain and symptom management
- Personal care assistance (bathing, dressing, etc.)
- Emotional and spiritual support
- Medical equipment and supplies
- 24/7 on-call support

Continuous Home Care
This level is provided during periods of crisis when a patient needs round-the-clock clinical care to manage acute symptoms. Care is delivered at home, allowing patients to remain in a familiar environment while receiving intensive support.
Services include:
- Continuous nursing care for symptom control
- Monitoring and crisis intervention
- Short-term, intensive support to stabilize the patient

Inpatient Hospice Care
When symptoms can no longer be managed at home, inpatient care provides short-term support in a hospital, hospice facility, or skilled nursing facility. The goal is to bring symptoms under control so the patient can return to home-based care when possible.
Services include:
- 24-hour skilled nursing care
- Medication management and pain relief
- Emotional and spiritual care
- Comfortable, home-like setting for both patient and family

Respite Care
Caregiving can be emotionally and physically demanding. Respite care provides short-term relief to primary caregivers by admitting the patient to a Medicare-approved facility for up to five consecutive days.
Services include:
- Temporary inpatient stay
- Continued medical and personal care
- Support for caregivers to rest and recharge
